Take the 2024 Community Health Needs Assessment (CHNA) Survey to ensure better health services for everyone in southeast Louisiana, especially the underserved.
What is it?
The CHNA is required to ensure hospitals are meeting the community's health needs and learning how health services can be improved. Residents of Jefferson, Orleans, Plaquemines, St. Bernard, St. Charles, St. James, St. John the Baptist, St. Tammany, Tangipahoa, and Washington parishes are encouraged to take the survey. This survey should take about 10 minutes to complete. Your answers are confidential and will not affect any services or benefits you are currently receiving. Thank you for your time!
